| 内容記述 | 吃音を伴う者において，クラッタリングや神経発達症など複数の症状を示すことが報告されている。また，発達性吃音を伴う成人(Adults with developmental stuttering: ADS)群では，聴性脳幹反応ピーク間潜時において左右の耳で有意な潜時差が示されている。この左右差は，自身の言葉を聴覚的にモニタリングする過程で発話タイミングの不一致を生じさせ，吃音の一因となる可能性が示唆されている。さらにADS群の約3割に側音化構音障害が観察され，語音弁別の問題も推定されている。そこで，ADS群11名と吃音を伴わない対照群14名を対象に，側音化構音の聞き取り課題からなるオドボール課題を用いて事象関連電位(ERP)のP300成分を計測し，その神経生理学的結果を分析した。その結果ADS群のP300潜時は対照群と比較し有意に遅延していた。特に/ki/とその側音化構音の弁別課題では，吃音の重症度が増すほど潜時は遅延した。したがって，吃音の機序として，聴覚モニタリング機能の問題に加え，吃音の重症化により語音情報を判断する際に遅延が生じ，時間的処理の問題が重複している可能性が考えられた。 | |||||||

| 内容記述 | It has been reported that people who stutter have multiple symptoms such as cluttering and neurodevelopmental disorders. Adults with developmental stuttering (ADS) showed significant interaural differences in interpeak latencies in auditory brainstem responses, suggesting a self-monitoring deficit as a potential mechanism. Furthermore, lateral articulation disorder was observed in approximately 30% of ADS participants. Therefore, we assumed that ADS may have speech sound discrimination disorder. Event-related potential (ERP) P300 measurements underwent during the oddball tasks, that were lateral articulation distortion speech discrimination tasks. Eleven ADS and 14 adults without developmental stuttering (control group) participated our research. We analyzed the aspects of their neurophysiological ERP and neuropsychological discrimination tasks data. As a result, the P300 latency of ADS were significantly delayed compared to control group. Particularly, in the discrimination task between /ki/ and its lateral distortion combinations, the latency was further delayed as stuttering became more severe. As a mechanism of stuttering, in addition to problems with auditory monitoring functions, it was considered that as stuttering becomes more severe, delays occur when judging speech information, potentially causing overlapping temporal processing issues. | |||||||
